can any one tell what we did wrong or was this some else going wrong?

None of the DTCs are clutch related.  I would look for some wires and associated terminal(s)  that were not properly reconnected.

I am going to replace the O2's today and see if that fixes the problem.

replace the 02's before the cats and clean MAF and she happy again.
