Before the web site was revised you could fill out your name, address etc and use drop down menus for track and run group and then print out the filled out form. This feature seems to now be missing. Is there a way to do this?

Hmmm.... I just check the old site on my local machine and it was just a PDF - the same as it is now. That could be a future upgrade though

I think it was actually last year's form that featured the drop-down boxes for the scheduled events. I think the issue this year was time, knowledge and availability of Adobe Acrobat Pro in order to set it up.

I worked with this a little more today. As downloaded, the form does not enable adding text. Right click on form, click "add text or signature", click on "add text".
